What should I bring with me on my first visit?
Please bring your physical therapy referral from your doctor, insurance card, and ID.
How should I dress for physical therapy?
Wear comfortable clothing that will allow your physical therapist to perform an examination on the area that we are evaluating and treating.
How long will my appointment be?
We reseve 1 hour time blocks for each patient. Treatment typically lasts 30-60 minutes based on the treatment plan established by your physical therapist
Where can I park?
We have free parking at both clinics!
Town Clinic – Parking is available at the end of Clayton St. You may park in any stall reserved for 770 Kapiolani Blvd.
Waipio Clinic – Parking is available outside of the clinic. You may park in any stall reserved 24 hours for Physical Therapy Associates (blue and white signs in stall # 42, 43, 93, 94, 94a).
What insurances do you accept?
We accept most private insurances, Medicare, Med-QUEST, workers compensation, and no fault insurances for Hawaii. Please contact us to verify your coverage.
How much will my copay be?
Copays vary based on your insurance carrier. You can contact our office to determine your individual copayment.
What if I don't have insurance?
We can offer physical therapy treatments at set fee due at the time of service. Please contact us for more information.
What is your cancellation policy?
We ask that you give us at least 24 hour notice if you need to cancel your appointment. If we do not receive timely notice, you may be charged at $50 cancellation/no show fee. Repeated cancellations or no shows may lead to the suspension of services.
